Media buttons are rendered on top of dialog

With the introduction of WordPress 4.0 the Add Media button and the tab riders for switching between editor types is rendered across the Credit Line Generator dialog window.

This impacts the functionality of the plug-in. Although you can still use the plug-in, you may not be able to see what you enter in some of the fields.

I will try and fix this as soon as I have the time.
