Map is not displaying and fields are not working correctly

I have just purchased the Pro Pack and have tried using the theme “Beside the Point 2012 (A)”. I have tried using the codes on my page http://www.oz-chill.com/distributor/australia/ but when you view the page, there is no map and also the formatting of the fields is all over the place.
Please help, is there something wrong with my coding, my setup, the theme I am trying to use, wordpress?
