Major problem – cannot get rid of plugin and password

Website is still asking for password even if is deleted.
I tried to delete plugin’s cache – no result.
I installed it again and I cannot change anything.

When I click the small lock icon the template breaks (look very strange) and I have to restore it.

PS: My site had a database restore.
