 * I put the MailChimp subscription button on this website:

   you can see it if you scroll down the homepage)
 * That is the “super slim form”.
 * But once the people opt-in I don’t get their name of website and that is what
   I need.
 * How can I add to the code the option that they _can_ write their name and website?
