Looking for: Plugin that stores and displays form information

Hi everyone!

I’m wanting to make a “healing list” form for my website that does the following:

1. Allows visitors to fill out a form that captures their name (optional), date, person who needs healing, and what their ailment is.

2. Displays all of the information ever entered on this form in a way that is printable and visible on the front end.

3. Allows me to delete old entries

Does anyone know of a plugin that will give me that functionality?
