Looking for plugin

Hi,

I’m setting up a software user group website. The software has several modules and not everybody is using all of them.
I’m looking for a plugin with which I can make a “input mask”, i.e. a page with names and checkboxes that the users can tick.
The plugin should allow users to select a module and display all other users, that use this module…

Is there such a plugin?
Perhaps I’m using the wrong search terms, but I wasn’t able to find something like that – although I’m pretty sure someone somewhere already programmed this…
