# Looking for file uploader plugin

 * Hello, I’m new to WordPress and am looking for a file upload widget/plugin with
   the following functionality:
 * 1.) Supports tiff, pdf, gif, png and psd files, or allows the admin to choose
   which files types can be uploaded
 * 2.) Supports multiple file uploads
 * 3.) Requires the user to fill in customizable text boxes (similar to a contact
   form) before the files can be uploaded
 * 4.) Sends an email to a specified address upon successful upload
 * Any ideas would be greatly appreciated!!
