Looking for contact information plugin to insert text via php

I’m looking for a plugin that might allow me to create an admin page where a user can input their contact information, address, phone number, or other data. Ideally, I would be able to echo any of these fields entered where ever i put a php call in the templates or in a post.

For example: if I want the user’s phone number to appear in the header, i would use a php insert to call the phone number into the header that would call the phone number inputted on admin data page. On a “Contact Us” page, I might use a php insert to input whatever address has been inputted on admin data page containing this data….

Can anyone point me in the right direction to a plugin that exists or one might try to modify?
