Looking for a specific type of form

    I want to create a page on my blog for graphic artists looking to get their work noticed. Basically what I want is a contact form that they can fill out with information such as:

    1. Name
    2. Photo
    3. Link to their blog or portfolio

    When they fill this form out and submit it to me, I want to be able to look over it and then when I approve it, I would like it to automatically appear on the page I designate for these Graphic Artists. It would be nice to have something like two or three table cells, side by side going down the whole page.

    Is there anything like this for WordPress. This is definitely holding my site up from going Live. Any help would be so greatly appreciated. Thanks 🙂
