Looking for a "Request" Plugin !

    hello
    i have a Movie Trailer Site
    and users every day send me an email for requesting a movie trailer
    and now i want to open a “Page” at wordpress so users can send me the movie details
    example:

    Movie Title: [box] <— enter the movie tittle!
    Comment: [box] <— enter a comment about the movie!
    Your email: [box] <— enter your email address!
    ———————–
    Only a plugin or a HTML document as a Page [not a widget]
    similer the “Contact us” plugin !
