Looking for a plug-in to sell memberships

Our local community theater sells year-long memberships and they’re looking to start selling them on their website, so I’m trying to find a plug-in that can handle taking the membership information and allowing the person signing up to pay for it online.

It has to be able to take a person’s name, address, phone number and email. They also need to be able to select their membership level (the theater offers four different types). The form also needs an area for a donation amount for them to enter.

It then needs to total what they owe and then direct them to PayPal.

The data also needs to be exported so that they can print out the list of memberships.

I have searched high and low and can’t seem to find anything that can do this. There are plug-ins that sell memberships to websites, but that’s not what they need.

Does anyone know of anything that can do this? The theater is a non-profit organization, so the lower the cost, the better.
