Same problem for me : after updated plugin, textfields has been removed.
i’ve fixed the issue by re-install the old version of the plugin.

Today’s update, 2.8.10, has not resolved the issue.
Yes, the issue is not fixed.
Wordpress system send by mail a fatal error message with WP user manager, after the update.

No extra textfiedl has been added.
login, password and connect/ login button only. nothing else.
Using siteorigin as pagebuilder and Vantage theme .
the loginform is a simply widget, before the update no problems, no issues.
After the update, the fieldsareas disapeared…
at now i use the old version of the plugin and i’ve no issues.

Update 2.8.11 has not resolved the issue. Textfields are not there in widget area to login. I still fix the problem by downgrading to 2.8.8.
Apologies for the delay here. This has been fixed in WP User Manager v2.8.12 that’s just been released.
