    If a given user has a broken Facebook avatar, either Facebook has updated/changed their URL scheme, or the user removed that photo from their Facebook account. You can prevent this from happening by using local avatar caching with the premium addon (which will copy FB avatars to your own server and point to that, rather than pointing to the original file on Facebook – thus even if the user deletes their profile picture, your server will still have a valid copy).

    The reason you don’t see this with users who just logged in with FB is because every login updates avatars – so if i.e. URLs or profile photos changed, it’ll be re-updated for that user who just logged in with FB, and thus not be broken.
