That is correct. They have to be active for the shortcode to work!

I changed the topic status as you suggested so I could update you on the issue.
After an extensive amount of time, we discovered that the issue was indeed a plugin conflict. In the final analysis, we had to make a choice regarding what plugin was most important for our needs.
Based on those needs going forward, UM was to be less important that others for our business model. It was removed and all of our forms are back to functioning as they should.
