Hello Chad.
I am not using the login shortcode.
I tried to add a filter once, I am afraid I might have damaged a php file then.
And – I have tried to reinstall twice. The first time, the unstall took very long and almost froze WP.
I reinstalled again – it looked liked it reinstalled fine, but my login form is still missing.
I fear that not all files have been reinstalled or some DB tables are not updated?

If you are uncertain about the files, you could compare against the list in the wordpress.org subversion repository (http://plugins.svn.wordpress.org/wp-members/trunk/). The login form is handled by wp-members-dialogs.php.
If you want to reinstall, I would suggest that you make sure to completely remove the plugin by using the delete option in the WP plugin panel. This shows as an option under the plugin after you have deactivated it. When you choose this option, it will remove all files associated with the plugin and clear out and settings from the db.
Then to make sure you get a clean re-install, download the most recent version, unzip it, and load it via FTP transfer.
