I am really sorry about the issue you are experiencing.
It is not normally a common issue for us. This kind of case must not have occurred.
We have checked and noticed that everything works fine on our end.
Most presumably, there is a JavaScript conflict with the Theme or one of the plugins.
When you click on the Add Question button and see the issue, please open the console with F12.
Next, go to the Console > Error tab and screenshot the errors you get there.
Send us the screenshot, so that we can see and suggest a solution for it.
