Can you do a hard refresh and see if that helps? Can you check your site in incognito mode of browser and see if that helps? Icon should show up but not sure why title has issues. Have you made sure you have set the title in the accordion instances created in the widget?
I have the same issue. But for me it is only in Firefox and in Opera (chrome and safari is fine) – on a mac. In Opera the icon just isn’t there, in firefox the icon looks like this.

update: hard refresh did the trick. Must been cache related.
Sinkadus – As I mentioned before I would check in incognito mode of the browser to ensure it is not a caching issue. I checked your site in Firefox and saw no issue (probably since I was accessing your site for the first time). Nice site, BTW.
Since we have not heard about this issue from past couple of weeks, I am assuming things are indeed working as desired and closing this thread as resolved.
