Layout problems

Hello.

I just checked the latest version of the plugin, 1.19 but I see there is still an error in the file “Insert_Reset_Password.php” as the form is still missing a closing div after the submit button. It may break some layouts (like in my case). I think no one noticed yet so maybe in the next version you can fix that too. 🙂
