keep getting a undefined error on user submit / and tool bar keeps disappearing

Hi,
Using the free vesion, and occasionally users cant submit because a pop up error message keeps displaying saying ‘undefined error’ – the weird thing is that when I make the page that form is on Private then Update, then visible, then press update – the problem goes away.
the same thing happens when the toolbar from the richtext editing area keeps disappearing. I havent updated any plugins – and some days it works other days it doesnt.
Any ideas?
