just won't work

    What are the exact steps I can take to reproduce this behavior under the following conditions:

    1. An unmodified, default theme such as Twenty Fifteen
    2. No other plugins active except for Awesome Surveys

    Will, i’m using catch box (child theme), but yes, i have many other plug-ins running. maybe it’s my security plug-ins causing the problem?
