Jologs Product Option for WP e-Commerce

I need a plugin that works similar to this:

This plugin appears to be old and I’m not sure if it is out of date or how to reach the developer. Does anyone have any experience with this plugin?

I need a mix of drop down and fill in the field options that will calculate additional fees if desired.
