Well, knowing that the checkbox works fine on default WordPress (as shown in the previous video), the only other things I can recommend:
– Send a link to your site’s form, and I would be glad to take a look to if anything jumps out as weird or whatever.
– Set up a new 5-minute install of WordPress, leave all at default settings, install only USP, verify proper functionality, and then begin recreating your site one plugin at a time, testing USP between each, until the culprit is revealed.

I think I found the issue: you renamed the setting, “Checkbox Name” from “usp_custom_checkbox” to “Zasady dodawania postów”, which is incorrect. As stated directly beneath the setting:
“Use only alphanumeric, underscores, and dashes. If unsure, use the default name: usp_custom_checkbox”
Please try changing the name so that it meets the specified requirements, and then try again should work fine. Note: do not include any special characters like ó, use only English characters for that particular setting.
