Is there a form plugin that does this?

I have the need for an online application type of feature…this is for grant applications.

Users need to go online and submit their information via this online form (also upload the occasional file such as a PDF)

I also have another group of users who need to login and “score” these applications and make comments.

Does anyone know of a wordpress plugin or combination of wordpress plugins that can accomplish this? A membership plugin combined with custom form? Any ideas?
