integrate form active campaign in genesis enews extended

    I have a question about the primary sidebar of my wordpress website (genesis enew extended). On that place of my website i integrated an form which refers to active campaign.

    The integration works well BUT in the form of active campaign there is a checkbox concerning a question for the privacy policy and there is a link to the privacypolicy as well. These 2 things are not seen on the form for my blogs on my website (for example: http://empowerjezelf.nl/wordpress-resources-at-siteground/). What do i have to do excactly to get the integration so that the checkbox and the link “privacybeleid” pup up in my form on my blogs? My goals is to have it working like the form on this page: https://takecharge.empowerjezelf.nl/life-coaching/
