Installing on a new server

Installing wordpress on with a new host on a linux server. Backed up my blog from my previous host on my drive. The activation is now asking for
Database Name — is this the folder name where wordpress sits on my server? ie “BLOG’
Database Username: do I use the same name as before when logging into wp-admin?
Password: do I use the same name as before when logging into wp-admin?

Database Host: localhost
