Installed & Activated — No effect on Form

I do not see any effect on the form display.
I followed these instructions:
Activate Plugin.
In Gravity Forms Settings, turn off \”Output CSS\”
In Gravity Forms Settings, turn on \”Output HTML5\”https://wordpress.org/plugins/gravity-forms-bootstrap-3-style/
