Insertions not woring correctly

Since updating to 2.6.0 and 2.6.1, my auto insertions are not working as usual.
My first insertion after paragraph 5 is now appearing after paragraph 20 on new posts. On existing posts, some are after para 5, but some are not.
My 2nd and 3rd insertions are also quite a few paragraphs further down the page than my settings.

But neither worked. However, I then gave one more TOC plugin a try, Lucky Table of Contents, and there is no issue. It might be because there is an option to use DIV markers instead of li/ul.
I don’t know if some popular TOC plugins are using similar code that doesn’t play well with the new version of Ad Inserter.
But as this is an issue that is taking a lot of time chasing, I think I will stick with the new plugin and see what happens.
