Insert/edit image not displaying/working

Couldn’t get the Insert/edit image icon to show/work when using v4.0.1 on WP 3.9, also i noticed that when i remove the show/hide rows from toolbar i got an error “thuis buttons how/hide other rows and you have a tool in” even if i don’t have any tool in.
