    This is usually due to an issue with your rewrite rules in your .htaccess file. Go ahead and double check that file and compare it to what rules are needed for WordPress multisite subdirectory here:

    It turns out that this was the fault of the Watermark plugin: http://wordpress.org/plugins/alti-watermark/

    I had done all the htaccess fixes and checks and it was driving me nuts. The Watermark plugin was active *only* on the base site (not network activated) and it completely broke all images in all sub-sites.

    Wow, so weird and not a very nice plugin..it probably wasn’t built/coded for Multisite. Glad you found the issue!
