Image Widget 4.2.1 not working in Lifestyle Theme

I’ve checked/deactivated all plugins and I am unable to add an image to the Simple Image Widget. I was previously able to. I’m not sure at what point it stopped working. http://www.kimberleyjones.com – Thank you.
