I would like to see a post URL when form submitted

I have been using this plugin for ages and still love it. However, there is something that would make my life a whole lot easier. I would love it if I could include the URL of the page the form was sent from. I have tried the [item parent_url] code, but that comes up empty.
I have to add that I am using it in combination with Themify’s Bizco theme and the form is places on a Testimony page. Which is pretty much like a post, just in a different location with some extra settings.

Any thoughts would be very, very welcome. That way I can keep all the info in one form instead of having to make a separate form for every separate page.
