I want to Auto create and publish a form linked to each post

Hello all, i want to know how to do the following:-
i have a 4.4 wordpress which list courses, and I will add each course as a post and it will have automatically its post ID, now i want when i press publish .. it creates a form from a pre-defined forms templates and link it to the post. So that each post has its unique form. So that course A is linked to a form A, and course be is linked automatically to the auto created and published form B. So that will save time instead of creating manually.
