Sorry for your case I am not sure, I am pretty sure it is a theme issue or a plugin conflict with other plugins.
You might to try the default theme to see if it works. I tried to debug but I could not figured out because the code is too way messy for me.
I see you have a lot of problems with PostRatings, have you tried other plugin to see if it works better for you?

I tried to reset the default theme and it works, so there is a compatibility problem with my theme couponpress.
I tried to delete the scripts that start on the page, css etc. … nothing works!
I tested many many many other plugins but they do not suit me, no logs by IP and ranking by the number of votes is impossible.
Visitors have the opportunity to cheat the votes and I am really frustrated. I was glad to see that your plugin provides these features.
On my site http://www.serveurs-rust.org I installed gdsr star rating but the IP logs do not work … useless plugin …

I see. I thought GD Star Ratings is pretty good, I always suggest that as an alternative because it is packed with features.
