After plugin activation go to Ebook downloads and create new ‘post’.
You can import your ebook in metabox.
When your ebook post is created, put widget to widget area.

Looks like you did not test your plugin. It generates brocken layout (it has non-correct opened and closed tags) and does not create table in db for name/email saving.
Can you share an example where is layout broken? It passes validator and not breaking a website structure.
If you have a problem with creating db tables, check your DB user rights.
And in general code is not tiny, elements like message errors and etc. too hard style, because they have not css classes. The <center> tag is not supported in HTML5 (it works, but not in specification)…
