I’m having the same issue, using WC 2.6.2.
It used to work very nicely with PayPal and an earlier WC Version but now I can no longer set it up.

Thanks for the update! Does this extra charge only work with the default Payment methods or should I be seeing extra charge fields on custom payment gateway plugin settings as well? (e.g. Authorize.net)?

The current plugin works with only BACS, Cash on Delivery and Bank Transfer. For other payment gateways, you may consider our premium plugin option.

I have two different sites with WC 2.6.6 in both of them. In one this plugin works and I see the settings, but in the other I cant see settings.
Any idea of what it could be?

You can check if both the sites are running on latest wordpress that is 4.5.4 or above and latest woocommerce.
If you have the most updated installation and still face the issue, it could be a conflict with some non compliant out dated plugin.
You can deactivate all the plugins except woocommerce and the extra charge plugin and it should work.
