I am having this problem too. I’ve used forms previously and received email notifications with no problem, but suddenly the notifications have stopped coming even though my “notify on form completions” is the same as it was before. Old forms, new forms, on Pages or Landing Pages–none are sending me email notifications.
I hope you can help us figure this out!
Please install the https://wordpress.org/plugins/check-email/ plugin and verify your site sends emails.
If it is not. You need to contact your hosting provider for them to enable phpmail on your server.
Also, log your email sends with https://wordpress.org/plugins/email-log/

I’ve set up “Set Email Response” from Forms, for “New Lead Notification” Template.
The problem is that now, the lead receives 2 emails: one from new lead notification(the admin does not receive it) and another from Get Response(double opt in).If i turn it Of, the Lead doesn’t receive any email.
