Hybrid Connect Not Working due to internal server error

Hello,
I’m trying to use hybrid connect, which is an opt-in form program. Its not showing on sites where I’m using the Hueman theme. I’ve been working with their support. They found a couple of internal server errors (status 500) and suggested I submit it to you to see if this might be something you could fix on your end. I’ll be glad to try and get you more information if you need it to figure out the solution.
