We had the same need, so we created a plugin for ourselves called Email Before Download and made it available for everyone else to download: http://bit.ly/dF9AxV.
It combines Contact Form 7 and Download Monitor so you can create any form you like, collecting whatever information you feel is required. You then have a choice of displaying the link to the download directly on the page or emailing it to the user. Hope it helps and let us know if think of any new features you’d like to see.

We certainly don’t want to pollute the forums, and this is the opposite.
We are excited to address the gap we feel has existed in WordPress for a long time and to share a solution with the community. Along with the likely chance that the same people with these old posts still have the need, these are the same posts we scoured to see if anything existed before we decided to spend the time to build our plugin — so like us, other users still come across these when looking around and now, because of our post, they might find a solution.
By the way, feel free to click the link we posted. It does actually point to the WordPress Plugins Repository, where we had already donated and published the plugin for free.

I’d love to use this plugin but contact form 7 stops our slideshow on the homepage from working. Too bad.

That is unfortunate. I hope you are able to get Contact Form 7 working. We are contemplating a solution in Email Before Download that doesn’t rely on it, but it is a nice plugin either way.
