    I am trying to use Form Maker in a custom theme with custom headers and very minimal inclusion of original WordPress code. I have tried to insert my form both using the toolbar button and using the short-code but neither work (all that is displayed is the short-code).

    When I compare my theme to a default theme (where the form works), I see that I am missing the call to the Form Maker Frontend Javascript library and the creation of a Form Maker object. What is the bare minimum I have to include in my theme so that the form works and where does that code go (functions.php, header.php)? Thanks.

    Dear Dave,

    Please be informed, that you need to include wp_head() in header.php file. Also you need to call the_content() function where you call page or post content.
