How to get url / title first Step

hi,

in my single post a has add shortcode : cfform1 (step 1)
and step 2 redirect to page: step 2

Now, how to show url/title single post in my email

i’m add special-mail-tags is: [_post_title] & [_post_url] but email echo: title+url page not is single post
