how to format phone number

There isn’t currently a [staff-phone-formatted] template tag in the Simple Staff List plugin. You’ll need to use the [staff-phone] template tag, and then add any html you may need around that template tag in your ‘Staff Loop Template’ area:
<p class="staff-member-phone">[staff-phone]</p>

Then you can use your .staff-member-phone { /* YOUR STYLES HERE */ } CSS selector.
