How to create my own style?

Should I be able to make my site look any way imaginable just by simply editing the CSS? Or do I need to edit index.php as well?
I would like to start with a fresh install, and edit the CSS as it’s given and be able to plug new styles in at a later time. But styles can’t be plugged in if index.php is edited.
