# How to Create a Test

   Plugin: Contact Form by Supsystic
 * Message: Hi, I’m trying to create a stress test with 10 questions. The default
   WordPress contact form could only take up to 7 questions. I downloaded Contact
   Form by Supsystic because it was compatible with my version of WordPress.
 * I’m having trouble setting up a field, ( I tend to get angry easily. )
    With 
   checkbox selection, [ ] yes, [ ] no
 * Is there a way to customize the contact form like this for 10 items? If not, 
   is there a plugin you can recommend?

    You can easily create contact from with such fields using our Contact
   Form plugin.
 * 1. For the creation of such field you need to choose checkbox list field type–
   [http://prntscr.com/bpghx8](http://prntscr.com/bpghx8)
    2. Then specify the name
   of field (name attribute for your field. you can use here latin letters, numbers
   or symbols “-“, “_”.) 3. Specify label for field – that your users will see on
   your Form right near your field. 4. Also you can set default value for your field,
   and one it appear on your site – field will be pre-filled with this value. But
   it is not obligatory. 5. Choose how display your field – in row or in column.
   6. And create “Select Options” for this field – specify name and label for it.
   In your case – “yes” and “no”.

 * I’m sorry. I was wrong about checkbox list field type. If you want to provide
   your users ability to choose only one variant for answer – you need to choose“
   Radio Buttons List” type for your fields.
    But all other settings of this field
   type are the same.
