How to add comments to a new page

I have been seraching for a simple way to do this:
I want to create a new page where I want the visitors to suggest a topic etc. using the comment template. I have seen some suggestions here to modify the page.php, but I don’t want the comments to be added to all pages. Could you suggest the best way to do it?
or
Is there a widget/plugin that I can install that I could call/insert from the dashboard?
