How to Activate Bootstrap Contact Form

I am building a site using a bootstrap-based theme and have added a contact form to the footer. However, the form doesn’t appear to be “activated” because nothing happens when you submit the form. Users are redirected to a 404 page.

There is clearly missing code that defines where messages are sent and, I assume, something to tie it into wordpress.

How do I do this? Can anyone point me in the diretion of a turorial?
