How Do I Get More Thank 2 Conditional Added Amounts To Appear At Oncce

    I added a conditional amount for:

    1)Select Form of Service:
    Regular
    Express(note: express attracts x2 of TOTAL charge)

    and:

    2)Charge of N30 shall apply for SMS notification sent?
    Please select
    Yes
    No

    The two are not appearing at once, can you pls be of help to me with this.
    Additionally, after placing an order, if i try to make new orders, i see part of the choices from above in /cart.
    Please can you be of help so choices get resetted after an order has been placed?
