How do I create a new tab in My Account (for Woocommerce)

I’ve struggled with this for some time, and cant seem to get any additional tabs added to the My Account page for WooCommerce.
Can anybody point me in the direction of where and what to add? Ive tried adding new custom templates to myaccount.php but it never comes out right – it just adds the new template to the existing tabs. How do I get this new template to register as a new tab with a link etc.?
