As far as I have understood after reviewing your message, you are asking how to add a review form using the plugin. Right?
If so, please try using the “WL: Product Reviews” widget on the template that you have set as the single product (product details) page.
This will add a review form where customers/users can leave their reviews.

That doesn’t create a review form. It’s empty space.

There’s no form for users to fill out a review on the product page when I drag it. The other widgets work fine.
I greatly appreciate you for sharing the screenshot of the widget you are using.
Please note that this widget won’t render the output (i.e. review form) in the editor mode.
First, add the widget to a template and save the settings. Afterward, check the product details page of your website.
Important: Before checking the product details page, please make sure that the template is assigned for the product details page.
