    Hello house, i intend to create a form where users can fill and also upload their passport photograph like the one here https://imgur.com/a/mYr1OTH

    Please which is the best plugin i can use to achieve this? i dont mind if is premium plugin or free.

    @tenzinchoenyi thanks for the reply. if i used contact form 7, how do i integrate it into UM? i want a link to be available in the member profile tab where users can click and take them to fill the form. please any idea how i will achieve this?

    I think you are using the Ultimate Member Contact Form plugin. I don’t know about this plugin but I will share documentation link. plusplugins.com/ultimate-member-contact-form-documentation/
    What theme are you using for this website?
    Can you share a screenshot of your contact form setting?
