I had the form as private. Have changed it to public so you can view it.
As for plugin I am just using one of the field available in forminator Pro. Maybe this a better topic for a forminator chat group. Kinda new to this so sorting it out.

As for your main query you can create multiple email notifications so that other users will get different data from submitted form.
For example admin will get all data, and end user who has submit the form will get only partial data:
